  • Co-payment(s), if required*
  • Any x-rays or other tests, such as an MRI or CT Scan, that have already been performed outside of OhioHealth. Please try to bring the actual films or a CD and a copy of the report.

* An additional co-pay is needed for X-rays at the Westerville and Gahanna location.

What is your cancelled appointment policy?

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask you to give us 24 hours advance notice, providing an opportunity for another patient to been seen.

What are your billing requirements?

Patients must pay any co-payments at the time of their visit. Patients are responsible to pay any amounts, such as deductibles, co-payments and co-insurance that their insurance company designates is their responsibility. To help facilitate making these payments, we offer an online bill pay option.

What do I do if my prescription runs out before my next office visit?

If you’re calling the office, please request all prescriptions and authorizations for renewals during our normal business hours when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
