Prepare for Your Orthopedic Appointment

Our office does not require a physician referral to be seen by our orthopedic surgeons; however, your health insurance company may require a referral. Please check with your insurance company prior to calling for an appointment.
 

For those who are ready to see one of our surgeons, we offer a variety of scheduling options for your convenience, Our team will make every effort to schedule an orthopedic appointment at a time that is convenient for you.
 

Please check with your insurance provider before making an appointment with OhioHealth Orthopedic Surgeons. This is to make sure that services are covered by your insurance provider.
 

As a courtesy to others who may require our services, we ask that you give us 24 hours’ notice when canceling an appointment.
 

Please arrive for your appointment 20 minutes early to complete paper work and for any possible X-rays that need to be taken.
 

Before scheduling an appointment for a Bureau of Workers’ Compensation (BWC) claim, specific forms need to be completed. Please notify our office if this is the case and we will work with you to maximize efficiency and save you time on the day of your visit.
 

Please bring the following items to your appointment:

  • Photo ID
  • Current insurance card(s)
  • Co-payment(s), if required*
  • Any x-rays or other tests, such as an MRI or CT Scan, that have already been performed. Please try to bring the actual films or a CD and a copy of the report.
  • Any copies of operative reports
  • New patient paperwork

* An additional co-pay is needed for X-rays at the Westerville and Gahanna location.
